Covid-19 Injury Illness Prevention Policy

All in-studio students and participants are required to sign and return this form before attending private training sessions or attending a class, at The Smart Body™.

DO NOT COME TO The Smart Body™ if

  • You have a fever

  • You have a cough or shortness of breath

  • You are feeling sick in any way

PROTOCOL AND SPECIFICS

  • Private clients are pre-scheduled to control the exact number of clients in studio at any time.

  • 15 minutes will be scheduled between clients to allow for proper disinfection and minimizing interaction between clients.

  • Clients are asked to remain in their vehicles until 5 minutes before their scheduled appointment time

  • All Clients are asked to promptly exit the studio on session completion

  • All students/clients will have contactless temperature taken at front door

  • Virtual Classes will continue to be offered. Check the schedule for class times

HAND WASHING/PERSONAL PROTECTION

  • Gloves will be made available for anyone who wants them

  • Clients will be required to wash hands or use supplied hand sanitizer both on entering the facility and leaving

  • Trainer will be required to wash hands before and after each client

  • Hand sanitizer will be stationed in every room and will be used between activities

  • Masks will be worn at all times by trainer and clients- NO EXEPTIONS

  • All students and clients will be required to provide their own face mask

  • Antibacterial diffusers will be used during the day throughout the studio

  • Individual/personal water bottles are supplied to each client and disposed of after visit. Please refrain from bringing in outside water bottles

  • Avoid touching your eyes, nose, mouth with unwashed hands

HOUSEKEEPING

  • Clients work directly with trainers allowing studio staff to identify and disinfect all equipment clients contacted during visit

  • Scheduling all Clients allows sufficient time (15 minutes) between clients for disinfecting and cleaning of contacted items.

  • Common areas (bathrooms, floor) will be disinfected 3 times daily (morning, afternoon, evening)

  • Clients are asked to bring a personal item bag suitable for shoes, jacket, and purses. Client personal item bag can be placed in a cubby, or kept near the front door. Personal item bag will supplied to Clients if needed. Socks are required in Studio. Pilates gripper socks are available for purchase (Toesox.com).

  • Clients are encouraged to minimize personal items brought into studio

Special Situations

Health Oversight Activities

We may disclose medical information to a health oversight agency for activities authorized by law. These oversight activities include, for example, audits, investigations, inspections, and licensure. These activities are necessary for the government to monitor the health care system, government programs, and compliance with civil rights laws.

Lawsuits and Disputes

If you are involved in a lawsuit or a dispute, we may disclose medical information about you in response to a court or administrative order. We may also disclose medical information about you in response to a subpoena, discovery request, or other lawful processes by someone else involved in the dispute, but only if efforts have been made to tell you about the request or to obtain an order protecting the information requested.

You have the right to request an "accounting of disclosures." This is a list of the disclosures we made of medical information about you. To request this list or accounting of disclosures, you must submit your request in writing to the address below. Your request should indicate in what form you want the list (for example, on paper, electronically). The first list you request within a 12-month period will be free. For additional lists, we may charge you for the costs of providing the list. We will notify you of the cost involved and you may choose to withdraw or modify your request at that time before any costs are incurred.

Right to Request Restrictions

Free 30 Minute Chat (phone)

NEW CLIENTS ONLY Your first consultation will be 30 minutes. During this time The Smart Body™ will answer any questions regarding Somatic Movement Therapy, The Smart Body’s approach, ask questions related to your health history and lifestyle, and discuss whether or not The Smart Body™ would be a good fit to aide you to your health and lifestyle goals. 

First Consultation Session

FIRST SESSION-NEW CLIENTS ONLY Should you decide to proceed, the next step is to meet for a private Zoom session. It will be 60-75 minutes long on a ZOOM number that will be emailed to you. During this time The Smart Body™ will review your health history, ask questions related to your health history questionnaire and lifestyle. After this appointment, an optimizing health program will be created around your specific needs. We will then schedule follow up ZOOM appointment(s) to go over your new program and how to begin implementing this program so you can achieve your health and fitness goals.

In Office Appointment for Physical Assessment & Wellness Consultation     

AS NEEDED BASIS Your first consultation will be 90 minutes. During this time The Smart Body™ will conduct a physical assessment and review health/lifestyle history. A personalized Health & Exercise Portfolio will be created around your specific needs and goals. We will then schedule follow up with Personal Training appointment(s) to teach you how to do your new movement-based program and go over your new optimizing health program.

Personal Training

PLEASE SEE COVID-19 PREVENTION POLICY All appointments are 60 minutes long and are in studio. During this time The Smart Body™ will instruct participants using the Dynamic Embodiment® approach to somatic movement education (Laban/Bartenief Studies, BodyMind Centering®, Experiential Anatomy, Developmental Movement Therapy, Alexander Technique, Hakomi Body Counseling, Massage Therapy), as well as Dance, Pilates and Yoga-based movement training, guided touch, and self-touch.

ZOOM Classes

All classes are 55 minutes long (unless stipulated otherwise) and take place on Zoom, along with other participants. During this time The Smart Body™ will instruct participants using the Dynamic Embodiment® approach to somatic movement education (Laban/Bartenief Studies, BodyMind Centering®, Experiential Anatomy, Developmental Movement Therapy, Alexander Technique, Hakomi Body Counseling, Massage Therapy), as well as Dance, Pilates and Yoga-based movement training, and self-touch.

CANCELLATIONS

  • If you are unable to keep your scheduled appointment, you must notify our office a minimum of 24 hours before your scheduled time or you may be charged for that appointment.

  • The Smart Body™ is not a medical doctor; we do not service medical emergencies. If you have a medical emergency, you must contact your primary care physician or dial 911!

  • The Smart Body™ will not diagnose any health or physical issues that you are presenting with. All exercise programs, lifestyle recommendations, and other recommendations are not prescriptions. They are suggestions of what you can do.
